About the Role

Haverhill Family Practice is looking to welcome a qualified and experienced clinical pharmacist into their busy surgery, to play a full and active role within their multi-disciplinary team.  The role is offered on a full or part time basis and this can be discussed at interview. 

The post holder will help patients get the best from their medicines by switching medications to agreed and approved protocols, improving repeat prescribing processes in general practice. 
 
Main duties include:
  • Working as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients using extensive experience and in depth knowledge
  • Delivering and developing pharmacy led medicines optimisation working with general practice and the wider multidisciplinary team, ensuring practice remains up to date.
  • Leading and promoting, supporting and facilitating medicines optimisation for specialist area in the wider medicines agenda ensuring best value, reduced harm and improved outcomes.
  • Leading and delivering evaluation and audit of service delivery for specialist area in order to ensure a high quality service and to further develop provision.
  • Supporting the prescribing lead with the medicines agenda within the practice.
  • Working to the principles of Medicines Optimisation and relevant up to date national guidelines and evidence base.
  • Providing clinical advice and expertise on medication.
About the Candidate
The ideal applicant will have strong experience within primary care and have the following skills and qualifications:
  • Masters degree in pharmacy (MPharm) or equivalent desirable.
  • Minimum of 2 years post qualification, or equivalent experience, including primary care and at least one other field of pharmacy (community or hospital).
  • Registered Pharmacist with General Pharmaceutical Council (GPhC).
  • Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience.
  • Good knowledge of NHS medicines optimisation.
  • Post-qualification experience in pharmacy (hospital, primary care or community), or working towards obtaining this.
  • Qualified independent prescriber, or working towards this. 
  • Non-judgemental attitude and ability to be flexible and adaptable.
  • Self-motivated with excellent communication skills.
  • Understand the need for patient confidentiality.
