About the Role
Fressingfield Medical Centre has an exciting opportunity  for a proactive and enthusiastic chronic disease nurse to join our highly effective, friendly and supportive practice team on a full or part time basis, working various weekday shifts between 8:30am and 6:30pm. 
 
This is a blended role with a strong focus on supporting chronic disease management alongside other duties to utilise skills of the post holder such as wound management, health promotion, women's health or medicines management.
 
Main duties include:
  • Reviewing patients with chronic disease i.e. diabetes, hypertension, heart disease, to implement and evaluate individual treatment plans to promote health and wellbeing.
  • Assessing lifestyle and medications and suggesting changes where appropriate.
  • As required, to diagnose and manage acute and chronic conditions.
  • Review of medications following an acute illness/admission
  • Oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
  • Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients’ health.
  • Prescribe medication for patients in accordance with PGDs and own scope of practice.
  • Assist GPs with minor surgery when required.
  • Implement vaccination programmes for adults and children.
  • Support patients in the use of their prescribed medicines or over-the-counter medicines.

About the Candidate

We are looking for a highly motivated and enthusiastic team player who can demonstrate high standards of clinical excellence and has experience working in a general practice environment. You will be experienced, dynamic and will have the drive and vision to shape the development of the role.

You will have the following qualifications/skills & experience:
  • Registered  with the NMC
  • Experience of working in GP practice setting.
  • Have specialist chronic disease management expertise in conditions such as asthma, diabetes, arthritis and dementia, although all similar experience would be considered.

The practice will consider applications from motivated registered nurses willing to undertake training to acquire new skills.
